Please plan to be
here for approximately one hour.
You will be asked to
fill out a bit of paperwork so please arrive 20 minutes early.
Please bring your
driver’s license or comparable picture I.D. for our records as well
as your automobile and health insurance cards.
You will be asked to
provide a list of all medications you currently take including
vitamin supplements. It may help if you make a list of them before
you leave home.
If you were treated
somewhere else and had x-rays,
MRI’s, or CT scans taken it would be most helpful if you
could bring them with you on this first visit. However it is not
absolutely necessary. Just make sure your paperwork indicates that
these tests were done.
If your injuries are
automobile-related we will also need your claim number. That is the
claim number your insurance carrier gives you.
Florida is a “no-fault” state meaning that no matter who caused the
accident, if you own a car, your auto insurance
coverage will pay for your medical bills according to policy
limits. That includes injuries you may sustain as a passenger in
someone else’s car, as a driver of someone else’s car, as a
pedestrian or while riding a bicycle. Your auto insurance will not
cover you while riding a motorcycle unless you purchase that
additional coverage.
If you do not own a
car nor live with a relative who owns a car you will be asked to
fill out an “Affidavit of Non-Ownership” at your first visit. We
have a Notary on staff to assist you at no cost to you.
Please note that we
